YOU MUST BE AT LEAST 21 YEARS OF AGE TO APPLY & LIVE WITHIN 30 MINUTES OF GREENWOOD, IN.

BEFORE APPLYING, THESE THINGS MUST BE COMPLETED!

-ALL of the animals in your home must be fully vaccinated, spayed/neutered, and your dogs must be on heartworm preventative to go any further in this application. We will be verifying this information with your vet.

IF YOU RENT: Before being considered as a foster, we must have your pet lease policy, please email it to mediacoordinator@tailsandtrails.org immediately after applying.

If Rent, we must receive landlord permission, in addition to this application in the form of a copy of the lease showing the pet clause or written permission that includes the landlord's name, address and phone number to mediacoordinator@tailsandtrail.org before foster dog can be released to applicant. This application will not be processed until permission is received.

Please provide Name and Phone Number of your vet. *Make sure your pets are current on ALL vaccines and that all dogs in the home are on heart worm preventative. We will be calling the vet to verify this information. Take care of this prior to applying please.
