By submitting the request form, I acknowledge that I will submit post conference required documents that include activity summary, tabulated evaluation and attendance records thirty days (30) after the conclusion of the event. Failure to do so will revoke contact hours awarded.
If event is to be held other than the requested schedule, a resubmission of request is required.
If this activity is approved, it is not an automatic approval for the same program when it is held on a different date other than what is indicated in this request.
I will resolve all deficiencies and correct recommendations (if any) 10 days upon receipt of initial response from PNAAF. Failure to do so will INVALIDATE this request. PNAAF will DISCARD or DELETE this request.
Furthermore, I attest that ALL members of the Planning Committee and ALL the speakers do not have any conflict of interest related to this activity. The contents of the presentations will be free of commercial bias.
