Joy Committee Adoption Contract

Please initial each box. This contract is legally binding, enforceable by law and agreed upon by both parties.

I understand that The Joy Committee Inc does its best to temperament test it's animals, test for aggression, health issues or any other problems, illnesses or problems, however there are times that unforeseen illnesses, agressive or destructive behaviors may arise. I understand and hereby release The Joy Committee Inc from liability, I hereby release The Joy Committee Inc and it's affiliates of any personal injury, or damage to any structure.

I assume full responsibility and ownership for my pet.

I understand that The Joy Committee Inc has provided me with my basic vetting to include rabies, 5way deworm and spay/neuter. I understand that any future vetting is solely my responsibility. I agree to keep my pet up to date on vaccines as well as heartworm prevention. I further agree to have any health issues that may arise immediately assessed by a veterinarian of my choice and I am solely financially responsible. I understand if I fail to keep up with the well being of my pet, the rescue may take my pet from me to ensure proper care.

I understand that my adoption fee is non-refundable in any situation.

I understand that if I need to return my pet, the Joy Committee Inc requires a 30 day written minimum notice and in some circumstances, I may be required to keep my pet for a longer period of time. I understand that The Joy Committee Inc is a 100 % foster-based animal rescue, there is not a facility for my pet to go to.

I agree that I will not surrender my pet to any animal shelter, rescue, animal control officer, person or facility without written permission from The Joy Committee Inc

I agree that I will not leave my pet outside unsupervised, chained, or in any dangerous situation that may harm my pet or others.

**For heartworm positive dogs only** All others may skip this question. I understand that my dog is heartworm positive and has started treatment, I agree to continue treatment as per The Joy Committee's Heartworm protocol. I agree that I will take my pet to any necessary appointments to treat his/her heartworms. I understand and agree to continue my dog on heartworm preventative after the treatment is finished.

***For puppies who are not spayed /neutered only** The Joy Committee Inc has a strict spay/neuter policy. I understand and agree that I will spay/neuter my pet by no later than 7 months of age, unless given written notice from The Joy Committee that this would harm or cause death to my dog. I hereby agree to take my dog to the vet of the Joy Committee Inc's choice and understand that if I do not get my pet altered my pet will be confiscated by a member of The Joy Committee Inc, and I may be responsible for any fees that occur after which time.

All vetting is done at Davis Animal Hospital in Pensacola. If you adopt a puppy that is too young to recieve his/her spay/neuter, you are required to have them spayed/neutered by no later than 7 months of age. This may be done at no cost to you. If you decide to use your own vet, we will give you a partial refund of your adoption fee.

The Joy Committee Inc makes not implied or express warranty with regard to this animal. The animal is adopted "AS IS". It is understood by adopter that should the animal cause personal injury or property damage after possession this is assumed by the individual pet owner, The Joy Committee Inc shall in no way be liable in whole or part for any damage. The individual hereby assumes all liability for the animal or any injury or damage that may be done by it.

Congratulations and good luck with your new pet. Please remember that pets need a "Cooling off" period to decompress from the shelter or to acclimate to the new environment. We recommend that you have your pet evaluated by a professional pet behavior analyst or trainer. Animals are what we put into them. If you are adopting a puppy, please understand the importance of training and working with them.
