I find it very helpful to learn your perspective on your child as we begin the college search and application process. I would appreciate it if you could complete this questionnaire at least a week ahead of our appointment. You will receive a copy via email once it has been submitted. If you have already completed a substantially similar questionnaire for your child's school counselor you may email or mail a copy of that questionnaire to me instead.

All of our children have strengths and weaknesses. What would you say are your child's strengths and weaknesses? Consider personality traits as well as natural intellectual, social, artistic and athletic abilities.

Does your child's academic record reflect her/his ability? If not, why not? Describe any special circumstances that may be interfering with her/his academic performance.
