  • I authorize Midtown Fellowship to deposit my pay automatically to the account(s) indicated below and, if necessary, to adjust or reverse a deposit for any payroll entry made to my account in error. This authorization will remain in effect until I cancel it in writing and in such time as to afford Midtown Fellowship a reasonable opportunity to act on it.

  • Please note that the updated bank account may not take into effect on your NEXT pay period cycle.  We run payroll on the 15th and the last day of the month.  So for example if this is submitted on the 10th of the month, payroll may have already been scheduled to run and would not take effect on the 15th of month payroll.  In this event, it will be effective on the last day of month payroll.  If there are any issues please contact wbutler@midtowncolumbia.com.

  • I claim exemption from withholding and I certify that I meet both of the following conditions for exemption.

    • Last year I had a right to a refund for all federal income tax withheld because I had no tax liability, and
    • This year I expect a refund of all federal income tax withheld because I expect to have no tax liability.

  • Please note that the updated housing allowance will take into effect on your NEXT pay period cycle.  We run payroll on the 15th and the last day of the month.  So for example if this is submitted on the 10th of the month, it will not take effect on the 15th of month payroll.  It will be effective on the last day of month payroll.  Also please note that new housing allowances are not pro-rated for previous pay periods.
