M O D E L   R E L E A S E 

By signing below, I (Client) am agreeing to allow Elizabeth Farnsworth to share images from my session on social media and for marketing purposes without compensation.

 

Birth Session

~Being on call within the month surrounding your due date

~Sitting by your side, whether in your home or the hospital after you reach active labor

~Documenting labor, birth, and 1-2 hours after birth

~Password protected image sharing gallery with over 100 hand edited images for downloading

~Prints available separately via an online gallery from professional printing lab

~A $50 credit toward prints

C O N T R A C T

50% as the retainer fee is non-refundable and due at booking. If the retainer is not submitted within 24 hours, this contract will be null/void and the session will be cancelled. The remaining portion of the session fee is due at the session. If the session fee is not submitted at the time of the session, a 24 hour window will be granted for Client to submit the balance. No images will be edited prior to balance being submitted. After such time, the images from the session are subject to being  permenantly deleted if balance is not paid. 

Images will be delivered 3 weeks after the date of the session. Images are delivered via instant digital download via Shootproof.  Discs and usb drives are extra and are avaiable for purchase. 

The content of the gallery is in descretion of Elizabeth Farnsworth. Elizabeth will select the best images and edit them to her descretion. Any further editing or culling will be subjected to fee.

Should anyone within your family/party become ill before your session, please let Elizabeth  know. Should a child attend a session and is visibly ill, it is within Elizabeth's right to reschedule the session immediately. 

(Due to the nature of my work, I am required to be very careful with what I allow myself to be around and what I allow into my studio. Thank you for understanding.) 

If for any reason including but not limited to: illness, emergency, weather and other acts of God Elizabeth Farnsworth is unable to attend the fresh 48 session, she will send a suitable back up photographer approved by her. If, for any reason including not limited to hospital restrictions, emergencies, special circumstnaces, illness or patient recovery photographers are prohibited, any fees and balances paid towards a fresh 48 will be applied to a newborn session. 

Elizabeth Farnsworth will retain the full copyright on all images (including digital files) and reserves the right to display, publish/etc. for any reason. Client is prohibited from further editing of images. Client is also prohibited from using photos in contests without written consent from Elizabeth Farnsworth. Client may not, for any reason sell their images to any third party. Client will be granted printing rights in writing for the images and is allowed to download and use digital files for printing purposes and their chosen vendor. I agree to provide proper photo credit to Elizabeth Farnsworth Photography for online and public display/usage.

 

L I A B I L I T Y

Elizabeth Farnsworth Photography is providing professional photographic services and is in no way responsible for the medical care of mother or baby. The client hereby agrees to not hold Elizabeth Farnsworth or Elizabeth Farnsworth Photography liable for any complications or injuries during the labor, birth, or postpartum period.

 

P E R M I S S I O N S

Client has gained any necessary permissions from birth professionals and facilities to allow for photographer to attend her birth.
