This form is for registered VWU student organizations. Requests from classes and athletic teams may be sent to Ginny Videll (email@example.com). Requests from off-campus groups may be sent to Amiya Keene (firstname.lastname@example.org).
Events are tentative until you receive approval and shouldn't be advertised prior. Please submit requests 2 weeks in advance (1 month for large events). Each time the Office of Student Activities approves/denies a portion of your request (ex. location, funding), we'll edit the 'status update' field in that portion of the form. You will receive an updated email from Jotform.
If you need to make changes or cancel your event, please send them to email@example.com. Do not edit your Jotform submission.
