VWU Fraternity/Sorority Interest Form - Academic Release and Critical Information

As a potential new member of a sorority/fraternity at VWU:

I understand that completing this form is a required step for all students interested in Fraternity/Sorority Life, but it does not guarantee an offer to join an organization.
I give permission for the Office of Student Engagement and my fraternity or sorority (chapter President or designee, advisor, and inter/national representatives) to view my high school and/or collegiate Grade Point Averages to verify my eligibility for membership and leadership opportunities.
I understand that membership in a fraternity or sorority comes with financial obligations such as annual dues. I may request information about such financial matters from the chapter(s) before joining.
I understand that members of fraternities and sororities are engaged in a variety of required and optional activities, to include campus and chapter events, civic engagement, and philanthropy.
I understand that hazing is strictly prohibited. If I join a chapter, I will commit to preventing hazing and I will report suspected hazing to the Vice President of Student Affairs or Director of Student Activities. Hazing is any mental or physical requirement, request, or obligation placed upon any person which could cause discomfort, pain, fright, disgrace, injury, or which is personally degrading. Examples include, but are not limited to, tests of endurance, physical brutality (whipping, beating, branding), forced or encouraged calisthenics, exposure to the elements, forced or encouraged consumption of any food, alcohol, drugs, or other substances, exposure to potentially dangerous or hazardous circumstances, activities with a foreseeable potential for injury, sleep deprivation, forced or encouraged exclusion from social contact, forced or encouraged conduct which could result in extreme embarrassment, panic, and degradation.
