  • Request/Change Employee ICT Access

  • Please note: Submission of this form will be received by ICT. ICT will then seek authorisation from the relevant Head of Service. Once authorised, your request will be dealt with as soon as possible.

  • Hardware/Software Request

  • Please obtain an initial costing / quotation for the requirement i.e. hardware or software. The process is :

    1. User fills in request form (Including business case if applicable).
    2. Pass to ICT who will seek quotations in line with procurement limits and guidelines.
    3. ICT will notify requester of cost and seek approval from the requester who in some instances will need Head of Service for approval.
    4. Purchase requisition raised by ICT Department on receipt of approved request.

    NOTE:  No order will be placed until the Head of Service has given approval after costing has been obtained and business case approved. The exception being anything costing under £25 - for example items such as standard issue headsets, keyboards and mice normally cost less than £25. In most instances ICT will approve but if in doubt they may need to seek approval from a relevant Head of Service.

    ICT will then seek authorisation from the relevant Head of Service if applicable e.g. the value is over £25.

    Once authorised, your request will be dealt with as soon as possible.

  • Notification Form of New Recruit

  • Please note:

    1. IT will seek authorisation from the relevant Head of Service and also from the HR Department.
    2. IT will process the request once authorised to do so.

    Also, If the new recruit requires a new PC or other PC equipment please fill in a hardware request form.
