Submit Charter and Investiture Supply Form to Cecilia Grizzell at the Ohio Conference by September 15.  Once received, the Certificate of Operation and the Advent Source Code will be sent to the Director.

Please enter each staff member or weekend helper.  Weekend helpers are defined as anyone who attends weekend events like fairs or camporees to assist with transportation, cooking, etc but are not regular members of the staff.

Your Club Staff list on Directors Corner will be updated from this form.  Only those staff/weekend helpers listed on Directors Corner will be cleared to attend conference events.
