For your claim to be considered, you must complete this form within 7 days of the incident. 3 photos of the damage must be included along with a description of how the damage occurred, the original cost of the item, item model number and 3 replacement quotes. Once submitted a member of our claims department will follow up with you within 2 to 3 business days.
