    By submitting this application, I affirm that I am a national AMWA member currently enrolled in a MD, DO, or combined degree program. I affirm that I have reviewed the application instructions and that the facts set forth here are true and complete. I understand that if I am awarded a grant, any false statements, omissions, or other misrepresentations made by me on this application may result in withdrawal of any funds conferred.
