Once finished, please click "Submit" below. This form will be submitted to Megan Martin, Interim Dean of Students/Associate Director of Academic Success, Avery Plummer, Student Affairs Coordinator, and Sylvia Pinto, Wellness Coordinator, for follow-up with the student.
Megan handles situations where students need tutoring, may need accommodations for a health/mental health issue or learning disability, or where excessive attendance issues persist after you have discussed them with the student.
Sylvia handles mental health and wellness issues, including food/housing insecurity, transportation, etc.
Student Affairs collaboratively handles code of conduct situations and issues in the dorms.
We can also refer students to resources for food, financial, transportation, or housing insecurity.
You will receive a follow-up email after we have discussed the issue with the student. If issues persist, you are welcome to follow up with us in-person or via email/Teams rather than submitting an additional form.
While students may ignore attempts at communication, the Early Alert process is a helpful tool in addressing challenges they’re facing. Your support and involvement is much appreciated!
