    In connection with my application for employment with Krintz Lawn Care, Inc., I understand that the Company will conduct background investigations, which may include consumer credit, criminal convictions, motor vehicle, and other reports. I authorize without reservation, any party or agency, including previous and current employers, contacted by this employer to furnish the above mentioned information.


    I understand and accept that as part of the application and employment process, and/or during employment with Krintz Lawn Care, Inc., I may be asked to submit to physical examinations which may include testing for alcohol and drugs, and/or be fingerprinted, all in accordance with law. By signing this application, I hereby agree to submit to such examinations and release all persons and companies from any liability arising out of such examinations, tests, and fingerprintings. I further agree that the examining person may disclose to Krintz Lawn Care, Inc. the results of same.


    If employed, I agree to conform to all policies, practices, and procedures of Krintz Lawn Care, Inc. and acknowledge that these may be changed, interpreted, withdrawn, or amended by Krintz Lawn Care, Inc. at any time, at Krintz Lawn Care, Inc.’s sole option and without any prior notice to me. I consent and agree that Krintz Lawn Care, Inc. shall have the right to search my personal property located on Krintz Lawn Care, Inc. property for purpose of investigating possible violations of Krintz Lawn Care, Inc.’s rules/policies.

  • Disclosure under Fair Credit Reporting Act and Consent to Procurement of Motor Vehicle Report for Employment Purposes Form

  • The undersigned herby authorizes Krintz Lawn Care, Inc. or its insurance agency Heiny Insurance, or its assigns, to obtain copies of Motor Vehicle Reports, which may be classified as a consumer report, pertaining to me for employment purposes and for use in rating and/or underwriting insurance for which the above named employer may apply and any renewal thereof, I understand that in obtaining such consumer reports, a consumer reporting agency may be used, and I do hereby authorize such use.
