This request starts the three setp process for completing the OYWA requirement for a background check. Please fill out this form completely. When you hit the submit button it will be sent to the OYWA support desk.

Step one: You will receive an email with documentation of your submitted request form.

Step two: After receiving this email response you will be required to mail a check for $25.00 made payable to Ohio Youth Wrestling along with a self-addressed stamped envelope for the process to continue.

Step three: Upon receipt of the check and self-addressed stamped envelope at the OYWA support deck, the background process will commence, with the sending of the final step process link sent in an email. It takes the background system time (2-3 days) after the final step process has been completed.

Please remember the process ends on January 1st.
