• Every Tiny Heartbeat Animal Rescue- Adoption Application

  • Thank you for your interest in adopting a dog from Every Tiny Heartbeat Animal Rescue!  Please understand that not all families and dogs are compatible.  Every effort is made to match a dog’s exhibited traits with characteristics you desire in a dog.  As such, adoptions are not done on a first-come, first-served basis. In the event we have multiple applications for the same dog and all applications are equal, we will most likely adopt the dog to the applicant having current or previous dog ownership.

     

    AS A RESCUE, WE RESERVE THE RIGHT TO DENY ANY APPLICATION.
    Every Tiny Heartbeat Animal Rescue reserves the right to deny adoption to anyone.

     

    To be considered as an adopter, you must be:

    At least 23 years of age and possess valid identification showing present residential address;

    Willing and able to invest the time and money required to provide proper care for a dog (estimated minimum annual cost = $950 per dog);

    Prepared to pay a $200 Adoption Fee (this includes the dog’s spay/neuter, microchip, vaccinations, heartworm and flea prevention, and any other medicines or medical procedures needed;

    Willing to allow us to do a home visit prior to adoption; and 

    Prepared to give us your veterinarian’s contact information if you currently own a dog or have owned a dog in the past.

    I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any ETH Rescue animal fostered by me.

    I certify that the above information is correct, and I understand that the information will be verified.

    I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Every Tiny Heartbeat Animal Rescue and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

    I accept full responsibility for the dog(s) actions at all times, and releaseEvery Tiny Heartbeat Animal Rescue from any liabilities or damages that may be incurred because of fostering such dog(s).

    I agree to have Every Tiny Heartbeat Animal Rescue complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

    I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to Every Tiny Heartbeat Animal Rescue and try to give Every Tiny Heartbeat Animal Rescue a 2 week period to try and find another suitable foster for the dog(s).
