For complete evaluation of your program, please include the following items:

1. All creative materials related to the partnership sale must be submitted in a request for a pre-qualification evaluation. These include:
- For retail sales: store signage, request cards, and any other advertising
- For direct mail: complete package of materials sent to consumer
- For Internet sales: all screenshots from the home page of the purchase location through confirmation of purchase/checkout.
- For telemarketing: full telemarketing script
2. Contract/agreement between the partner and the publisher/agent noting the agreement for program execution and remit of monies from partner to publisher/agent and fees charged from partner to publisher/agent.

3. Copies of the materials the consumer will use for processing of the subscription and/or request for refund. Please include step-by-step instructions of what the consumer will be required to follow.
