Square Request Form

Use this form to request a Square device code, which will be linked to your EWB-USA account. This form must be submitted at least 5 business days before your event. 

 

Square login information will only be sent to the Gmail account created by EWB-HQ for your chapter. (example: denverprofessional@chapters-ewb-usa.org)

 

EWB-USA HQ will issue a Square device code to a chapter officer upon request. Prior to receiving the device code, the requesting chapter officer must agree to the following terms and conditions.

Terms and Conditions:

• EWB-USA’s Square account is intended for event and auction fundraising. Classy, EWB-USA’s donation platform, remains the preferred vehicle for soliciting and receiving donations for chapters.

 

• Upon registration, the Square device will be tied to EWB-USA’s designated bank account and the requesting chapter’s General Fund account. Square devices cannot be tied to a University bank account.  

 

• Square devices are for event transactions with a $5 minimum.

 

• Chapters are responsible for obtaining and maintaining their own Square device. If a device is lost, the chapter must notify EWB-USA HQ as soon as possible.

 

• Chapters are responsible for covering the Square processing fees, which are taken out of each Square transaction automatically and are 2.75% per transaction.

 

• Misuse of your square device will result in shut down

 

• If you use a Square, the purchaser will receive an email receipt from Square. A tax deductible receipt is not provided. If they want a tax deductible receipt, they can’t use Square, they need to use your Classy page and make a donation. 

 

• If you are using more than one device, you must get more than one device code.

 

• You must fill out a customer profile for every customer.
