Girls Club Charter Request Form
Please indicate how many membership cards you will need for your club(s). Membership cards will be mailed to the address given. Once your request is submitted, it will be processed by the International Girls Ministries Department and mailed to your State/Regional Coordinator. She will sign your charter and mail it to you. Please ask your pastor to sign it as well. Charter certificates are suitable for framing and display. The church’s name, chartering date, and assigned number are on the certificate, as well as the signatures of the International Girls Ministries Coordinator, the State/Regional Girls Ministries Coordinator and the Pastor.
