Dear SLATS Applicant:
Thank you for your interest in St. Lucie Alternative Transporation System (SLATS) paratransit service for perople with disabilities. SLATS service area includes all of the St. Lucie County.
St. Lucie Alternative Transportation System (SLATS) ADA service is offered in accordance with the Americans with Disabilities Act (ADA) of 1990. The ADA Paratransit service is for persons with physical, cognitive, emotional, visual or other disabilites which functionally prevent an individual from using the fixed bus route system either permanently or under certain circumstances. The Treasure Coast Coneector fixed route is fully accessible. other accomodations such as bus stop and route announcements make using the Treasure Coast Connector buses possible for many people with disabilites.
All applicants seeking eligibility for the ADA Paratransit Service must go through an eligibility determination process to determine if an applicant can use the Treasure Coast Connector bus route.
Those individuals that are certified eligible will be classified in one of the following categories:
Unconditional status is assigned to person who are determined unable to ever independently use Treasure Coast Connector buses.
Transitional status is assigned to persons who are determined capable of using acessible Treasure Coast Connector buses but cannot do so at present because of a temporary disability.
Conditional status is assigned to persons who are able to use Treasure Coast Connector buses most of the time, but would, under certain circumstances and for certain trips, be prevented from independently using Treasure Coast Connector buses.
To enable us to determine your eligibility, please complete the enclosed application explaining how your disability prevents you from riding regular Treasure Coast Connector fixed route buses. You may complete the form yourself or have it filled out by a person familiar with your condition. Should you need Spanish or Creole formats of the application, please call the office at (772) 464-7433, Option 1.
If you qualify for St. Lucie Alternative Transporation System (SLATS) transportation, you will be mailed your St. Lucie Alternative Transportation System (SLATS) membership card. The membership card is valid for three years from the date of issue. Your membership card also identifies you to receive a discounted rate on the fixed route system once you show the card to the driver.
If you do not qualify for St. Lucie Alternative Transporattaion System (SLATS), a letter will be mailed to you explaining the reason and advising you of the procedures to follow if you wish to appeal.
If for any reason, you are not contacted within 21 days after submitting your completed application, you will be entitiled to receive St. Lucie Alternative Transportation System (SLATS) service at least until you receive a definite decision on your application.
