The following questionnaire will help us choose the right puppy for you and insure that our puppies have TERRIFIC homes and families to love them. Please be open and candid in your responses. We want to find the puppy that best meets your expectations so that he/she can be your best friend for life. Your assistance in completing this questionnaire will help us to place the right puppy in your home. All information will be kept confidential and used only by us. We will contact you once we have reviewed the questionnaire. Submission of this questionnaire is not a guarantee that a puppy will be available. We reserve the right to screen all applicants. This will also give us a chance to chat with you so we know what you are looking for in your new puppy. When placing our puppies. we will look at the individual lifestyle of each new potential home and what activities you would like to participate in with your dog and match the best puppy to that home
