If applicant has prior health care work or volunteer experience, the applicant MUST upload documentation of job description/or volunteer experience, length of service, and hours per week from supervisor on agency letterhead. Applicant must be working for a minimum of four (4) months in the past five years. Volunteer experience must be for a minimum of 40 hours in the past two years. Prospective Student Observation Form

Prospective Mid MRI students are required to observe at least four hours in a hospital medical imaging department in order to evaluate MRI as a career. The observation requirement may be waived in lieu of experience working in a medical imaging setting. If seeking a waiver of this requirement, briefly describe previous experience in a medical imaging setting on a document and upload below.

Personal Statement

A brief account of:
1. Significant experiences, activities, and employment.
2. Your reasons for considering a career in Health Sciences.
3. How your medical imaging department observation related to your reasons for selecting a career in Health Sciences.
4. Your plans and aspirations for the future.
The personal statement is to by typed on a seperate document, should be a maxiumum of one page in length, and uploaded below.
