By submitting this Mac Checklist I acknowledge that I am responsible for my own data and email backups, and that I need to seek assistance if I need help with backups. I understand that the IT Help Desk provides data and email transfer during a new Mac upgrade or installation as a service, and that this includes formatting of the computer. Both formatting and transfer may result in data loss that is not retrievable. I understand that any items not included on this form and not on my backup will be lost. I understand that some older hardware may not be compatible with more recent versions of OSX. I understand that any programs requested that are out of date will be upgraded. I understand that freeware programs unrelated to my job function and unlicensed software will not be installed, in accordance with University Policy 870. Refer to desktop Service Level Agreement for hardware requirements and supported software.
