I, {name4}, understand that Graduate Student Organization (GSO) Grants are subject to all of the University of Hawai‘i’s purchasing, fiscal, and student conduct policies. I also understand that these funds may only be used for the purposes, stipulations, and/or special conditions approved by the GSO. I understand that should my reallocation application not be approved, the original award is still valid and will have two weeks to submit the reimbursement packet. Should my reallocation application be approved, I understand that if I do not submit all reimbursement packet materials within three months of reallocation approval date (Grants and Awards Committee reviewed date) or within three months of end of activity as specified on the Reallocation Application, whichever comes last, I forfeit my award. In the event that the research cannot be conducted, completed, or finished as proposed, the GSO will not provide me with a reimbursement. Furthermore, if I am unable to provide the proper and complete documentation for reimbursement, such as original receipts, boarding passes, or proof that I have paid for the items, I understand that my reimbursement may be impacted, including partial funding or a denial of the award. I agree to submit the Grant Award Completion Report to complete the reimbursement packet (Please see the GSO website www.gso.hawaii.edu for detailed information regarding all necessary documentation).
I, {name4}, have read and understand the information regarding the awards and the reimbursement processes, including, but not limited to the Graduate Student Organization Grants and Awards Policies and Reallocation Policies documents. I understand that I must allow at least two weeks from the time of my application before I get any information about the status of my application. I understand that GSO Grants are given out as reimbursements and that I must allow 12 weeks from the time I have submitted a complete reimbursement packet to receive a check.
