After this form is submitted, the Production Project Manager will schedule a meeting or phone call with you to discuss the details of your needs. If your event requires anything beyond what this form asks, please fill out a Production - Event Request Form found as part of the Event Planning Form. Please Note: This form is for a room to be prepared for use. If you need a Production Team to support the entirety of your event, please use the Production - Event Request Form.
