Mount Horeb Softball Club Annual Membership Form
The purpose of the Mount Horeb Softball Club (MHSC) is to promote the growth of girls’ softball from youth through high school ages, at all levels of competition, within the Mount Horeb community. The development of sportsmanship, teamwork, and respect for others are key priorities. The MHSC will promote fundamentals of the game while fostering a fun and positive softball experience.

Your annual membership will help us to achieve our goals and improve the athletic experience for our kids. As a member, you will have the opportunity to vote at regular, annual, and special MHSC meetings. Your vote will prioritize club projects, select fundraisers, and amend club by-laws in order to sustain the success of the MHSC.

Annual membership is not required for your child to participate in softball. Even if you don’t wish to become a member of the club, please provide contact information so we can include your family in communications about softball in the Mount Horeb area. Please note this form does not serve as registration to school softball or other softball teams.
