Provide as much information as possible and relevant paperwork for the team to assess your surrender request. Please note that the demand always exceeds our resources and we have limited capacity to take in surrenders, especially during kitten season (Oct - April).

In order to submit this surrender form, we require certificates of microchip, vaccination and desexing if applicable. Not providing this documentation at this time will affect our ability to assist you. Maggie's Rescue is run by volunteers and as such, we do not have the resources to contact your vet to obtain basic paperwork for your animal. If this paperwork is unavailable, please email admin@maggiesrescue.org.
