  • UC Blue Ash College CPAS Re-Entry Petition

  • If you were provisionally admitted to UC Blue Ash College, you were accepted based on the condition that you participate in the College Program for Academic Success (CPAS), earn a minimum number of credit hours each semester, and achieve a minimum GPA each semester. Failure to meet any of these requirements at the end of your first or second semester makes you ineligible to attend classes at the university, and your provisional admission is revoked.

    Re-entry to the program and the college is not automatic or guaranteed. After one year, you may be considered for re-entry if you can explain in your petition why your circumstances have changed, what skills you have gained, and how your are ready to meet the challenges of college. Prior to the term in which you would like to return, you must complete the online petition form and submit the petition form and any accompanying documents by the deadline noted on the form in order to be considered for re-entry.

  • Be sure to have all materials prepared for your submission. It will not be possible to save this form and return to complete it at a later time.  If you need time to write your responses, consider typing them into a different document, i.e. an MS Word file. Once you are finished typing your responses, you can then copy and paste them in the appropriate areas below.

  • Deadline for Spring 2022  Re-entry

    For re-entry in Spring 2022 - Submit by 11:59 p.m. on October 1st, 2021.

    • Regular re-entry is available after one year of not attending UC. 
    • Early re-entry (less than one year of not attending UC) is only considered under extraordinary circumstances. 
  • Re-entry to future terms

    If you are preparing to petition for regular re-entry to a future term, please check back to this petition form closer to the term you are looking to return in. Part of the submission process requires including details about how you have taken this time to improve skills and be better prepared to succeed.

  • Re-Entry Information

  • NOTE:  The term you have selected to petition for re-entry is less than a year from the term when your admission was revoked. Petitions for early re-entry prior to the end of the one-year waiting period will only be considered for extraordinary circumstances, such as death in the family or extended, documented illness.   Denied petitions will count towards a student’s maximum of two petitions for re-enrollment.  If you have questions about this, contact your CPAS Advisor.

    The Petition Review Committee requires documentation related to your particular circumstances. Documentation, such as evidence of completion of educational programs, special training, or work experience, may be relevant and support your application. If you are applying for early re-entry, your documents should also include documentation of the extraordinary events that prevented your successful completion of the CPAS requirements, such as a doctor’s note or evidence of death in the family.
