to participate in activities sponsored by Grace Church from June 1, 2019 to August 31, 2020. Events may include but are not limited to: Scavenger Hunt, Youth Group, Overnights, Camping, Retreats, Middle School Camp, Student Impact Week, Worship Camp, Momentum, Student Leadership, Mini-mission trips and Service trips. This permission slip is valid for all activities during the above specified time unless written notification of revocation is delivered to Grace Church, 7600 Liberty Road, Powell, Ohio 43065 by the undersigned.
The undersigned parent/guardian understands that Kalahari 2019 involves certain physical activities that could result in injury to the child and by registering for the event, the parent/guardian assumes the risks associated with those activities; provided that the activities are overseen by Grace Church leaders, etc. in conformance with normal church policies and are conducted in a reasonable manner.
Statement of General Health
My child is in good health and I consider him/her capable of participating in the events sponsored by Grace Church Student Ministry. I understand the events may include but are not limited to: Scavenger Hunt, Youth Group, Overnights, Camping, Retreats, Middle School Camp, Momentum, Student Leadership, Mini-mission trips and Service trips.
Videotaping and Still Photographs
Video and still photographs may be taken during these events. This authorization form constitutes permission for my child’s participation in the videotapes and/or still photographs, which may be used for future promotional efforts, including the Grace Church websites and other church publications.
Code of Behavior
You are representing Christ at Grace Church during sponsored events. We expect that you will display a mature and responsible behavior that for many years has been a trademark of youth and adults of Grace Church.
Participants are expected to adhere to the following:
- All participants are expected to arrive on time and stay until the program ends at the assigned time, unless a parent/guardian delivers written consent to the Student Pastor that you will be leaving early.
- All participants are expected to demonstrate common courtesy and respect at all times. Inappropriate language/behavior will not be tolerated.
- Socializing should always be done in public areas. When away on retreat/camp no guys in the girls’ rooms and no girls in the guys’ rooms. Public Display of Affection (PDA) is not allowed at any time.
- Dress should reflect the value of modesty. Writing on clothing must be appropriate.
- Littering, especially improper disposal of chewing gum, is not permitted.
- The possession or consumption of any alcoholic beverage and/or possession/use of any illegal drug by an individual is not permitted.
- Smoking is not permitted.
- Weapons and/or drug paraphernalia of any kind is not allowed.
- If under the age of 18, prescription drugs in the original prescription bottle with instructions for dosage must be given to the Student Pastor for storage and distribution.
- Infraction of these rules can mean immediate dismissal with no refund. Parents and/or legal guardians are responsible for any costs associated with the dismissal, including, but not limited to, all travel expenses. Participants will be responsible to the local authorities as well.
I understand and agree to the Code of Behavior. I also understand and agree that at the time of an infraction requiring my dismissal, I am responsible for my removal from the premises and any cost involved. If under the age of 18, I also understand and agree that my parents or guardian will be notified at the time of an infraction requiring my dismissal.
