Part 1: Application Process
This is the process by which bookable access facilities can be requested:
- Purchase a ticket
- Obtain a ticket reference number
- Completed Access Requirement Form (only valid with a ticket reference number) and any supporting evidence
- We will endeavour to respond within 5 working days to confirm your requests
- We will then send you a confirmation email
Submitting your form:
Upon completing this form, it will be automatically sent to the Parklife Access team.
Closing date is Friday 26th May 2017 @ 18:00

If you are having trouble uploading your evidence, please feel free to send this separately in an email to email@example.com.
You will be asked to supply supporting documentation as part of this application form. This helps to speed up the process considerably.
Please scan in or take a picture of you evidence. Please feel free to black-out any information that relates to amount of benefit paid or health conditions etc. that might be on documents.
Automatic qualification applies to those able to supply the following documentation dated within the past 12 months or with valid date covering the event:
Front page of DWP letter with PIP letter
Front page of Attendance Allowance letter
Blue badge accompanied by driver’s license photo card and/or passport photo page
Evidence of being registered as severely sight impaired (blind)
Recognised Assistance Dog ID card
