This form needs to be completed for each team you are playing on based out of Apex or Morrisville!

 

 

CONSENT TO PARTICIPATE: I acknowledge all risks of injury and death and affirm that I am willing to assume responsibility should injury result from them.  I also agree to follow all rules and procedures of the porgram and to follow the directions of the staff of this facility and general Parks, Recreation and Cultural Resources Department policies and procedures.  Furthermore, in return for the opportunity to participate in this program, I agree for myself, my heirs, assigns, executors, and adminstrators, to hold the Town's Parks, Recreation and Cultural Resources department, its employees, or its agents harmless for bodily injury or death resulting from this program, and to release those parties from any liability for damages resulting from my injuries or death.
