NEB File Closure Request

Please use this form if you permanently want to close your file and/or do not intend to take the NEB exams anymore. Closing your file means that you will not have to pay any annual renewal fees anymore and you will not be eligible to take any exams with the NEB. You will have the option of getting your documents returned to you or to have them destroyed permanently.

If you decide to go through the NEB process at a later date after permanently closing your file you must apply as a new candidate, pay all fees and support your complete documentation again.
