At Baxter Veterinary Clinic we want your pet's grooming experience to be pleasant and enjoyable. Some pets respond differently than others to grooming. We promise to do our very BEST to make grooming a positive experience for your fur baby. Should your pet need a sedative to decrease stress and/or achieve the groom you request, do you authorize the use of such medications?

Certain hair types can become matted more easily than others. Matted fur can be extremely painful to remove, and in some cases, skin infections may be present beneath the matted fur. If your pet has matted fur that cannot be brushed out without causing significant pain or injury to your pet, do you authorize our groomer to shave these areas?

During the course of grooming health concerns such as ear infections, skin infections, broken toe nails, growths, etc. may become apparent. If any medical concerns are present do you authorize our veterinarians to evaluate your pet?

Please tell us of any medication your pet is taking.

Some pets have more sensitive skin than others. Our groomers are very careful with pet. In some cases nicks, cuts, or rashes may occur when working close to the skin to remove the undercoat. We assure you that every effort will be made to prevent such skin irritations.

General Consent:

With my signature, I agree to indemnify and hold Baxter Veterinary Clinic harmless from any and all liability arising out of the performance of any of the procedures referred to above.

We will contact you using the information provided below to arrange a time for you to pick up your pet. Please indicate if you require a certain discharge time so that we may try to accommodate your needs.
