  • How to Use this Service Order Form

    1. The customer should complete the form. The form should only be completed after all pertinent information and logistics have been negotiated between the parties. 

    2. Once submitted, a copy of the form will be emailed to the site admin, the customer, and the service provider.

    3. The service provider will receive two links: one acceptance and one rejection. He or she has until the Service Response Deadline to accept or reject the agreement using the links provided in the email they receive. The customer will not receive these links.

    4. All parties will receive confirmation of the rejection or acceptance. At that time, a professional relationship exists between the parties and each party has committed to complete his or her tasks associated with the agreement, including meeting deadlines. If issues arise which prevent the satisfaction of the agreement, the party experiencing those issues has the responsibility to notify the other party immediately as is reasonable. The parties may renegotiate or cancel the agreement at that time. 

    5. If the project is accepted, the admins will create an automated reminder for the deadline to be sent one week prior to the final deadline. 

    6. If the customer fails to send the necessary files to the provider by the submission date, the agreement is void. (This does not mean that the service provider cannot or will not accept the late submission; it means they do not HAVE to accept the late submission. If the parties agree, the agreement may continue with no break in the service timeline. If new deadlines are established, return to this page and complete a new agreement.)

  • Please create an "Agreement Number" for this project. The agreement number is an identification number that will match this project with the rejection or acceptance your provider will complete. It can be any combination of letters, numbers, or special characters of your choosing. We suggest your initials followed by a three digit number, but it can also be as simple as the title of your work. (e.g. MAC001 or My Book Title)

  • When selecting a date for final acceptance or rejection of this job offer, you must allow for at least 24 hours due to the international make-up of our group. It may be lunch time where you are and the middle of the night where your provider lives. The BEST practice for completing this form and the acceptance and rejection of the offer is to complete the form while you are in direct contact with your provider via messenger, phone, or text. When this best practice is followed, acceptance and/or rejection can occur immediately.
