When we call your veterinarian to inqure about your previous or current pet(s) care managament, it is exremely important for us to have your pets information exactly as your veternarian has it on file. If we can't verify your veternary care, we can't approve your application.

Some veterinary offices requre your permission to release information. Please notify your vet that we will be calling and give them your permission to speask with us.

That all the house hold members are listed on this adoption application and that each member residing at this address has never been convicted of animal cruelty, abandonment, neglect, or abuse.

That I am at lease 21 years of age, and that I am willing to spend the time and money necessary to provide medical treatment, proper nourishment, care, and indoor shelter for my cat.

That if relevant, I have followed all the required rules of pet ownership set forth by my lanlord or condo association.

That if I can no longer keep the animal, I will contact Greenmore Farm Animal Rescue and make arrangements to bring the cat back when kennel space is availble. I realize it may take several weeks for the rescue to make space for my returning cat.

Yes, I am aware that my adoption donation is non-refundable. My donation is a gift that keeps on giving. By taking home my cat, I have opened up space and resources for another cat to be rescued from a "High Kill Animal Shelter."

When multiple applications are submitted on any given cat(s), applications are considered on a first come basis. The application that best suites the needs of the cat is given preference. If your application is approved, please do not schedule an appointment until you are ready to adopt your cat. Greenmore Farm Animal Rescue CAN NOT HOLD OR BOARD CATS.

Please note: We reserve the right to refuse adoption to anyone. Please be advised that we will not adopt to persons who mislead or fail to provide accurate information on the application.
