Dog Adoption Application 2017

Please read through everything completely and answer every question completely. If it is not filled out completely, your application will automatically be discarded. Also, adoptions are not first come, first served. We choose based on the best fit for the dog. If you are chosen as a possible adopter, we will notify you. Approval can take as long as a week. Due to a low number of volunteers, we are unable to contact you if you are not chosen to adopt the animal. Please do not fill out an application if you are planning on going on vacation within the next 2 weeks. This is unfair to other applicants, and also holds up our foster home as we wait for you to return.

We also want to offer the option to add a harness lead to your adoption for an additional $30. A harness lead is a comfortable, escape proof leash/harness combo. They are considered no pull harnesses as well. Would you like us to include a harness lead for your dog?

Any reclamation of the dog due to incorrect information or breech of contract will be done at no cost to Power of the Paw. I give my permission to contact my landlord, personal references and veterinarian and allow for a release of my veterinary records to Power of the Paw. I understand that applications not fully completed with correct and required information will be discarded and will not be processed. I agree to pay the adoption fee of the dog, in full, at the time of adoption and am aware of all payment expectations. I also authorize Power of the Paw to use any photos taken after and/or at the time of adoption for future social media posts and promotions.
