Welcome to Discovery Bay International School. In order to welcome and understand your child's needs and preferences and help them settle in quickly, we would be grateful if you could take some time to tell us about your child.

To simplify the entering of data, we have divided the form into tabs. You can navigate through the form by either using the tabs at the top or by clicking next at the bottom of each page.

Please be aware that a recent student passport-sized photo is also required whilst completing the form. The photo should be colour, must be less that 1MB in size and of a recognised photo format (e.g. jpg or png).
