Edna Lucille Crumpler Scholarship

 The winner will be notified on or before May 15, 2023. By submitting this form I am attesting that I am a High School student in my senior year with plans to attend an accredited institution of higher learning in order to complete my Associate or Baccalaureate degree. I currently reside in Staten Island, NY or attend a High School located in Staten Island, NY.

In addition to submission of this form applicant must have all required supporting documents postmarked by deadline. Students are required to submit official High School Transcript, 3 letters of recommendation. Recommendations are required from the following sources one(1) from Parent/Guardian and two(2) from either Teacher, Counselor, Mentor and/or Coach on letterhead. At least one recommendation must be from Teacher/Counselor. Applicants must also supply a photo with documentation. Likewise, all correspondence and award agreements will be generated from the personal information submitted below. So, please use proper spelling, capitalization and punctuation.

 

Optional Informations :

Information regarding ethnic origin and race are optional. The information is requested for post-scholarship report generation only and in no way affects your selection for college scholarship awards. However, providing this information may increase your opportunities for scholarships where donors have asked that priority consideration be given to students meeting certain demographic criteria (such as minority status, parent's place of employment, area of study, etc.)

 

 

Applicant must provide acceptance letter, and documentation to support enrollment of college/university in order for funds to be issued.  Proof of enrollment documentation of winning recipient must be submitted by August 1, 2022. Failure to comply with these stipulations will forfeit scholarship award. Award funds vary based on support of donors. Funds are distributed via check and will be mailed on or before August 15, 2022.

Elibility requires that applicant must be entering an accredited institution of higher learning pursuing a 2 Year or 4 Year degree and attended High School as a senior during the 2021-2022 school year. Student must begin college by January 2023 to be eligible. High School seniors graudating prior to June 2022 or after June and by August 2022 must provide diploma with supporting documentation.

SAT and/or ACT scores are not required for scholarship eligibility. However, providing scores can increase the likelihood of your selection as compared to other candidates.

All scholarship applicants reporting an SAT/ACT score will be asked to provide official verification.
