This form provides permission for my youth to participate in the activites of the New Lisbon Christian Church's Impact 429 Youth Ministries. These activities include all activites which will take place at the church, as well as activities which will take place off church property. I understand that these activities may or may not require my youth to ride in the church van to the to the destination. 

I understand that my child must uphold a certain standard of behavior while participating in activites sponsored by New Lisbon Christian Church. I understand that if my child acts inappropriately during these events, it will be my responsibility to come and bring my child home. Inappropriate acts include: use of drugs, tobacco or alcohol, fighting, vandalism, continued behavior which is distracting from the group or is causing an unsafe situation, and or any other behavior which is deemed inappropriate by the director of this event, youth ministry staff, or youth ministry volunteers.

I hereby release, hold harmless and discharge New Lisbon Christian Church youth ministry staff, and youth ministry volunteers from any and all liability, claim, loss, damage, cost or expense. I waive such claims against the orgranization or any such person, arising directly or indirectly from or attributable in any legal way, to any action or omission to act of any such organization or person in connection with the execution of youth events. 

Furthermore, I allow New Lisbon Christian Church and its staff and student volunteers to seek emergency medical attention for my youth should that be required using the information provided in this release form and will assume all financial responsibilites that may occur during their medical treatment.
