Consent to participate in the RX4Health Study – University of Miami Miller School of Medicine
Purpose of the Study: The Rx 4Health is a health prescription program but it is also a research study aimed at promoting health and well-being through physical activity and nutrition education. It is intended to justify that an after-school program for kids or a low impact aerobics program for adults can have a positive impact on health and well-being.
Description of Study Procedures: The study has been reviewed by the Institutional Review Board (IRB) and will be monitored by Dr. Sarah Messiah, principal investigator, and the University of Miami’s Human Subject Research Office. With your consent (parent/guardian), you agree to allow basic performance and anthropometric measures (height, weight, blood pressure) to be taken at least twice during the program or at the request of the physician.
Confidentiality: All collected data will be saved in an electronic database maintained by the University of Miami. That database can only be accessed by the principal investigator and study coordinator. All information collected will be assigned a personal identification number. Your name will not be included in the database or associated in any way with data collected.
You will not receive payments or reimbursements however your prescription allows one year of free participation in the intervention program. No other compensation will be provided.
