      The KC Society s objective is to be a strong, positive influence on all who are developing their personal habits and values. A volunteer’s example is most important in achieving this goal.       I hereby give my permission for my child named above to participate in all assigned field trips that KC Society institutes. I, also agree, that my child named above can be transported in the vehicle of KC Society’s choice.I further agree that, in the event of an accident, illness or any other circumstance requiring medical treatment, such treatment may be procured for my son/daughter without financial obligation to KC Society.

    • I (We), the undersigned parent, parents or legal guardian of a minor that is participating in the YELI program, do hereby authorize and consent to any x-ray examination, anesthetic, medical or surgical diagnosis and treatment and emergency hospital care which is deemed advisable by an is to be rendered under the general or special  supervision of any member of the medical staff and emergency room staff licensed under the provisions of the Medicine Practice Act and on the staff of any acute general hospital holding a current license to operate a hospital from the State of Florida Department of Public Health. It is understood that effort shall be made to contact the undersigned prior to rending treatment to the patient, but that any of the above treatment will not be withheld if the undersigned cannot be reached.

      I do, hereby, grant to KC Society the unlimited right to use and/or reproduce photographs and videos in any legal manner for the purpose of promoting KC Society and its programs. I also agree KC Society may interview and/or photograph my teen and/or me in relation to any or all coverage of KC Society. I further understand that by signing this release, I waive any and all present or future compensation rights to use the above stated material(s) unless otherwise negotiated in a separate contract, that supersedes this agreement.

    • Waiver Agreement 

    • WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT

    • In consideration for receiving permission to participate in the KC Society Youth Emploment and Leadership Initiative (YEL Initiative).  I hereby release, waive, discharge and covenant not to sue KC Society, its officers, servants, agents and employees (hereinafter referred to as "releasees") from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the releasees, or otherwise, while participating in YEL Initiative, or while in, on or upon the premises where YEL Initiative is being conducted, while in transit to or from the premises, or in any place or places connected with YEL Initiative.


      In signing this release, I acknowledge and represent that:


      A. I have read the foregoing release, understand it, and sign it voluntarily as my own free act and deed;


      B. No oral representation, statements or inducements, apart from the foregoing written agreement, have been made;

      C. I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
