College Bound Success Registration

I pledge, as a parent/guardian of a College Bound Success Participant, I will:

 

• Help my child fulfill the requirements needed to be successful within the program
• Support my child's goal of attending college
• Return phone calls or emails from the College Bound Success staff

I understand that KC Society is not responsible for my safety during or in connection with my attendance at any College Bound Success program, event or meeting. I hereby release, discharge, indemnify and hold harmless

KC Society and its officers, agents, employees, staff and volunteers, and any other persons or entities acting on its behalf and their successors or assigns, against all claims, demands and causes of action whatsoever either in law or in equity, related to injury, disability, death or other harm, to person or property or both, arising from my participation in or transportation to and from any such College Bound Success program, event or meeting.

I also recognize that pictures taken of students during College Bound Success events and meetings may appear on our website or in publicity materials. Written permission must be received to stop the use of a participants picture

I agree to uphold the following commitments.

I will:

• Treat the College Bound Success staff, participants, volunteers, and guests with respect and courtesy.
• Return phone calls from College Bound Success staff.
• Consistently attend the 1.5 hour site meetings once per week for the full 8 weeks
• Call or e-mail College Bound Staff by 1:00 p.m. on the day of the workshops to let them know if I will be late or absent and let him/her know the reason for the absence (NOTE: we will fulfill the same obligation to every participant to ensure success).
• Notify College Bound Success staff of any changes in my address, phone number, and/or e-mail address.
• Try my best and stay focused during the weekly meetings on the goals of graduating from high school and enrolling in the college of my choice.
• Work to maintain or achieve at least a B average in school.
• Keep an open line of communication by asking for assistance from the College Bound Success staff if I have any concerns or questions about what I do not understand 
• Notify College Bound Success Staff and my partner as soon as possible if I must discontinue participation
• I understand that College Bound has the right to remove me from the program if:
  • I do not meet the above expectations.
  • I have excessive absences, which will negatively impact my participation in the program.

I hereby authorize College Bound Success to obtain a copy of my report card for each advisory period, my transcript,my Standardize Test Scores, PSAT, SAT, and/or ACT test scores. The report card will be collected to ensure that I am meeting the College Bound requirements of keeping my grades consistent or showing improvement. I recognize these strict guidelines will also serve me by increasing the probability that I will be accepted into an institution of higher learning.

By signing this Electronic Signature Acknowledgment Form, I agree that my electronic signature is the legally binding equivalent to my handwritten signature. Whenever I execute an electronic signature, it has the same validity and meaning as my handwritten signature. I will not, at any time in the future, repudiate the meaning of my electronic signature or claim that my electronic signature is not legally binding.
