  • I, the applicant, do hereby certify that I am the legal owner, or duly authorized agent for the owner of pets under the care of Eastside Animal Hospital.

  • Vaccines and Internal Parasite Testing:

    In order for your pet to stay with us, vaccinations must be administered in accordance with our wellness care protocol.  If your pet did not receive vaccines at this facility, you must provide us with documentation from a veterinarian that specifies vaccinations administered and intestinal parasite testing.

  • External Parasites:

    For the health of all of our lodging / grooming animal guests, Eastside Animal Hospital has established a flea-free/tick control policy.  ALL ANIMALS using our lodging/grooming services must be up to date on a veterinary approved and veterinary dispensed flea prevention and tick control product.

  •  Lodging and Your Pet’s Health:

    • Eastside Animal Hospital operates a veterinarian supervised lodge.  We take the health and safety of your pet seriously.  Please advise us of any health concerns upon entry to our facility.
    • If your pet is on medication, this medication will be administered by trained personnel.
    • Please bring your pets prescription medication in the originally dispensed child proof container. If medications need to be filled or refilled, applicable fees will be added to your invoice.
    • Please alert us to any special medical needs (e.g., diabetes, kidney disease) that are relevant to your pet.  We offer transitional care services for “fragile” and “senior” lodging guests. Additional service fees apply;  ask us for details.
    • We want your pet to enjoy us.  Please alert us to any special handling requirements.
    • When your pet is admitted to our facility, you are verifying that your pet is free from any contagious disease and he/she has not bitten anyone within the past 10 days.
    • In the event that your pet(s) become ill and EAH is not able to reach you or the emergency contact number, we will pursue diagnostics and treatment.  Applicable fees will be added to the invoice.
    • In the event that your pet(s) become critically ill, you authorize EAH to transfer your pet to a 24-hour emergency hospital of EAH’s choosing.
    • Eastside Animal Hospital is not staffed 24 hours a day.  We make every effort to ensure that each patient is comfortable and stable prior to our late evening departure.  By checking below you acknowledge awareness of our business hours.
  • Statement of Lodging Policies:

    • For your convenience, a day’s lodging is charged for every night your pet is with us regardless of drop-off or pick-up times.
    • Pets can be picked up between 7:00am and 7:00 pm Monday and Tuesday, 7:00 am and 6:00 pm Wednesday-Friday, and Saturday 9:00 am – 2:00 pm
    • We are closed on Sundays
    • Personal items maybe left; however, we are NOT responsible for lost or damaged items.
    • We provide bedding for your pet.  However, if you prefer to bring your own bedding we will wash it once after it is soiled.  Subsequently, your pet will receive our bedding during that stay.  We are not responsible for damaged bedding.
    • We strive for a stress-free fear-free lodging experience.  We strongly recommend adding “extracurricular” activities (playtime, strolls, fitness fun package, etc) or can we add link here? to improve your pets time away from you.
    • We make every effort to make your pet’s stay at our lodge a home away from home experience.  However, we are not responsible for often unavoidable conditions in multi-animal environments such as, but not limited to, weight loss, hair loss, rough hair coat, respiratory infections, diarrhea, and odor upon pick up.
    • Please leave us YOUR EMERGENCY CONTACT INFORMATION with each visit to our spa.
  • IF APPLICABLE: I choose to house my family pets together. I understand that Eastside Animal Hospital is not responsible for fights that may occur between my pets. If fighting occurs, pets will quickly be separated and appropriate additional lodging fees will apply.
