By signing this permission slip you acknowledge that your child may actively participate in Princess & Tiaras Spa Celebrations on Wheels activities which may include the following:
Your child will be receiving finger and toes nail polish application, the use (not limited to) perfume, shimmer powder, sparkle hair spray, lotion, nail stickers, nail files etc. Your child may also have a makeup application with the use of eyeshadow, blush and/or lipgloss. Your child may also have a glitter tatto with the use of body glue and color sparkles. The use of party props, clothing, accessories etc. Your child will have her pictures taken / video recordings during the event. Photos and videos may be uploaded to an online gallery and/or social media and may also be used for promotional purposes. Princess & Tiaras Spa celebrations cannot be held responsible for how photos are shared. During the event, your child may be going up and down the Spa on Wheels, may be dancing and jumping inside the Spa on Wheels. Princess & Tiaras Spa Celebrations is not responsible for any damages, accidents or injuries that may incur while under our supervision.
By signing this permission slip, you assume all risks and hazards incidental to participating in the spa/party activities and waive, release, absolve and indemnify and agree to protect, defend and hold harmless Princess and Tiaras Spa Celebrations, their owner supervisors, participants, hosts and drivers for any claim arising out of any injury/medical injury to your child to the fullest extent. You will further grant permisssion to use without recompense, any photographs, videotapes, audio recordings or any other recordings for promotion of Princess & Tiaras Spa Celebrations.
