HALL OF FAME - Nomination Form

The USTA Southwest Tennis Hall of Fame was founded in 1991 to memorialize members of our Section, both players and non-players, whose exemplary character, achievement and contributions to tennis are worthy of the highest honor and recognition.

Hall of Fame selection is regarded as the most prestigious honor awarded by USTA Southwest.

GENERAL ELIGIBILITY REQUIREMENTS
1. All territories of the Southwest Section shall be included in the Southwest Hall of Fame.
2. A member of USTA Southwest must nominate the candidate
3. Age Requirement
Player Category: Candidates must be 35 years of age + to be considered.
Non-Player Category: Candidates must be 45 + to be considered.
4. The candidate must be of good moral character.
5. A person who is deceased may be considered for induction.
