I hereby release Downtown Dance, employees/independent contractors from all liability for personal injury, illness, or property damage occurring on or off the studio’s premises. I authorize Downtown Dance to seek medical treatment at the nearest medical facility and they may call paramedics and discharge me/my student to an ambulance if I am not able to authorize it in case of a medical emergency. I certify that my student is in good health and capable of participating in physical activities.
I hereby give permission to Downtown Dance to take and use photographs for promotional uses for the studio.

I understand that payment is not refundable after June 1, 2017.

The prince is giving a ball! Join our second annual summer dance camp &
show. It will be magical dancing to Disney and Rodgers & Hammerstein’s
Cinderella music, and we’ll have fun making memories with friends. Play a
part by picking from one of the following classes:

Cinderella Camp Participation Includes:
-Light Blue T-Shirt (or iconic color shirt for main characters like pink for step sister, Anastasia) *dancers will need black bike shorts.
-Class/character hair piece or accessory
-Class dance & Finale dance in the show
-15 hours of classes, 6 hours of cast rehearsal, Mon June 26 Popcorn, Pillow & a Cinderella Movie event, Dress Rehearsal & Show. Show Location TBD.

Mon June 26- Popcorn, Pillow, & Cinderella Movie plus in-studio rehearsal (optional movie for mice, mandatory for other classes)
