Completing, Signing and Submitting the Online Documents
The PARENT/GUARDIAN must initiate the process by clicking the link below to open the Contestant Contract and Scholarship Rules & Regulations.
A new window will open for the PARENT/LEGAL GUARDIAN to enter their FIRST Name, LAST Name, and Email Address.
The PARENT/GUARDIAN must also enter the TEEN Contestant's FIRST Name, LAST Name, and Email Address. NOTE: The TEEN's Email address must differ from the PARENT/GUARDIAN's.
The PARENT/GUARDIAN may proceed to enter the required information onscreen, upload the attachments and electronically sign the document.
Once the PARENT/GUARDIAN submits the document, the TEEN CONTESTANT will receive an email with a link to complete her portion of the document, electronically sign and submit.

Finalizing the Submission Process
AFTER both of you have electronically signed and submitted the Contestant Contract, you must return to this page and submit the form below.
Once we receive this form, we will review your submission and let you know if the documents meet the requirements so we can forward to the Miss Ohio Board of Directors for approval.
