ALLERGY, DIETARY & SPECIAL REQUESTS - Due Wednesday, April 9th

We need specific information regarding allergies, dietary, and medical needs while you are in Mansfield for Miss Ohio Week.

We have host homes (where you will stay at night) and chaperones (who are with you during the day).

Each host home will house two contestants (sometimes three), and those contestants will be assigned one chaperone for the week. Many factors go into determining the actual pairing, including your group, performance position, allergies, talent, etc. Returning contestants may request a host home and chaperone. However, we cannot guarantee that we can honor your choices.

If there is another contestant that you would like to room with for the week, you may list them below. There is no guarantee that you can room with someone, so you might want to list multiple names. If there are contestants that you would prefer NOT to room with, please list them as well.
