New to CREDO Mobile? Use this form to request your credit of up to $350/line in early termination fees or remaining device payments.
Before you submit your form, please make note of the following:
- In order to receive your credit, you must have been a CREDO Mobile member for at least 30 days and submit your request within 6 months of enrollment with CREDO.
- You will need your CREDO Mobile Account ID. You can find this number by logging into your Member Services account.
- You will need to have a digital copy of your last bill from your previous provider. On the bill, all we need to see are the relevant line items calling out either early termination fees or device financing/installment payments. If you receive a paper bill, we recommend using your phone to photograph the pertinent pages.
