How did you hear about MO Min Pin Rescue?
I will return all pets to MMPR should I no longer be able to foster. I agree to give at least one week notice if I must return a foster animal.
I will immediately report top MMPR any dog that is lost, stolen, ill, or dies while in my care. Should my foster dog need medical attention, I will use only a veterinarian specified by MMPR. All veterinarian visits must be approved by the rescue.
I will provide regular updates to the foster coordinators. I understand that an MMPR has the right to review the care given to all foster animals and remove any animal from a foster home if necessary. Upon cessation as a foster home, I agree to surrender any and all properties belonging to MMPR
I agree to abide with the MMPR FOSTER HOME GUIDELINES, and understand that all foster animals must be adopted through the MMPR adoption procedures only.
I agree that I will not hold MMPR responsible for damages, illness, or injuries, which may occur while fostering a pet(s).
I understand that as a foster parent, I get first chance at adopting the foster dog. I also understand that unless previous arrangements have been made, I am expected to pay full adoption fees.
By submitting this application I understand that all foster dogs are the property of MO Min Pin Rescue and, should it become necessary, I/we will relinquish the foster dog back to a representative of MO Min Pin Rescue.
